Reports to Care Manager > Case Leaders Summary of role To guide, direct and support our Support Workers in their care practice, enabling them to provide the very best service to our clients in a way that respects both dignity and promotes independence. The role is very much a supporting and monitoring role and works alongside our Case Leaders. Principal responsibilities of a Senior Support Worker Guiding and mentoring Support Workers Collecting information and documentation from a clients home for updating documentation in office. Supporting the Case Leaders in introductory visits to the clients. Supporting with quality assurance such as spot checks, observations, audits on files. Conducting annual client and Support Workers surveys to monitor client satisfaction. Play a pivotal role in enhancing and promoting relationship between our client and our Support Workers. Provide ideas and activities to stimulate and engage the clients and families. On occasion, respond to emergencies and
provides guidance and support to Support Workers. On occasion and when required, participate in the emergency on-call phone. Support assigned clients with aspects of personal and clinical care once trained and signed off by either our Case Leader/Clinical Lead. To help in the promotion of mental and physical activities of clients through community access activities and initiatives. To understand our clients and build professional relationships by reading the clients bespoke Care Plan. In order to ensure our service is consistent and delivers the elements that matter most to our clients- as stated in the Care Plan and reflected on our call management system (People Planner & ACP). To read and write reports and be involved in clients reviews and training activities. To always adhere to all Caremarks policies and procedures. To report either to the Case Leaders or Care Manager any significant changes in the health or circumstances of a client. To encourage the independence of clients and
monitor overall comfort and well-being of both clients and carers, wherever possible. Our Support Workers, including Senior Support Workers, are expected to include support that would reasonably be given by members of the clients family and is not to include tasks that would normally be undertaken by a trained nurse. Flexible & reliable Caring, sensitive and supportive to the needs of others Ability to use own initiative Ability to communicate effectively Excellent recording and reporting skills Resilient and is able to work under pressure and can manage changing priorities
